On diagnosis of my late mother from 95-2009, Our GP encouraged us to have early family meeting - which helped us and also Consultant was the same. We were allocated an Outreach Nurse who was available for contact. Through to end of life - Our wishes were observed.

Clare, do you have any information that you could share on Advance care planning? When I spoke with my GP about this she said as I had dementia I needed specific information and advice
